The Resilient West Belfast App is a population health planning tool to develop access to services & promote wellbeing across the population of west Belfast.
The app has been codesigned with service users and providers to provide an easy way to navigate services and access information at the touch of a button.
The app has been developed under the main headings of population health planning.
Social determinants of health including education, housing, transport and leisure.
Health behaviours & lifestyle – smoking cessation, alcohol consumption, diet & nutrition etc.
Places & Community – the impact of our built & social environments on our long term health outcomes.
Integrating health & care system support services for those with chronic conditions.
In this app, you will find information on organisations that provide support within the West Belfast area, including an introduction to what they do and how you can contact them. There is also information on services available to help people cope with a range of issues, including addiction, poverty and mental health. A distance guide is also provided so you can see how close you are to these essential services.
